Heart & Health provides compassionate and comprehensive support for adults re-entering the community from a mental hospital. Our program is designed to be a nurturing, supportive housing environment that helps individuals find stability, rebuild their lives, and achieve lasting wellness. We offer three tiers of personalized care to ensure each resident receives the right level of support at every stage of their journey.
Each resident receives a personalized care plan tailored to their specific needs, including mental health support, medical care, and social services. Our dedicated staff are available around the clock to provide close monitoring, support, and crisis intervention if needed. Residents have access to a range of therapeutic services, including individual counseling, group therapy, and occupational therapy to support their mental and emotional well-being. Regular monitoring and management of medication to ensure compliance and address any side effects or concerns.
Residents who wish to continue support will receive assistance with educational opportunities such as GED support and help with securing employment and managing work/life balance. They will receive continued support and monitoring of medication to ensure adherence and address any ongoing concerns or adjustments. Dedicated case managers will regularly assess progress on-site, provide support, and adjust care plans as needed. Residents live in semi-independent housing units with access to communal spaces and community activities to foster a sense of belonging and reduce isolation. Residence will participate in peer-led support groups to share experiences, build relationships, and provide mutual encouragement.
Residents have access to long-term, independent living arrangements with the security of knowing they can stay indefinitely. Residents will continue to receive psychiatric appointments to monitor mental health, adjust treatment plans, and ensure ongoing support. Residents will have access to Lighthouse mentors who provide guidance, support, and encouragement, helping residents navigate challenges and achieve personal goals. Residents will have opportunities to participate in community activities, volunteer work, and social events to build connections and foster a sense of belonging. Case managers will also offer periodic check-ins to address any emerging needs, provide resources, and support continued growth.
