At Heart and Health, we believe everyone deserves accessible healthcare, nutrition and housing. Our mission is to drive systemic change by delivering integrated, transformative solutions that address both medical and non-medical factors that impact ones health.
Who we Serve
We partner with churches and community organizations that fight against food insecurity, health disparities, and homelessness in Texas. Through partnerships and collaborations with primary care physicians, licensed psychiatric providers, and housing facilities, we act as a bridge for individuals and families seeking stable housing and/or healthcare.
